We at Community Hospice, Inc. work with patients and their families to achieve a safe and comfortable transition, self determined life closure and effective grieving.
Hospice care is provided to those who, in consultation with their physicians, have decided that supportive rather than curative care is the goal. The focus of hospice is comfort and quality of life. Traditionally, hospice care is provided at home, where the patient is cared for by family members, friends or hired caregivers. When this is not possible, arrangements can be made for hospice care to be delivered in a skilled nursing facility, a residential care facility or other settings.
Patients and their families and friends experience many conflicting emotions when faced with a life threatening illness — emotions such as fear, anger, loneliness and anxiety about the future. The hospice team helps both the patient and family cope with the experience of a life-threatening illness on all of its dimensions: physical, practical, emotional and spiritual. All members of the team have unique knowledge and expertise that they contribute to the process. It is the role of the hospice team to present and discuss options with the patient and family, and to assist them in making informed decisions.
We are dedicated to helping patients and their families make decisions about their own care. The team provides coordinated and comprehensive care to address the physical, psychosocial, and spiritual concerns associated with the end of life process.
Hospice care is patient-centered and patient-directed. It is one of the best examples in health care of collaboration by an interdisciplinary team, dedicated to meeting the needs of the patient and family. In consultation with the patient, their loved ones, the primary caregiver, and the patient’s physician, The hospice team prepares an individualized plan of care based on the patient and family needs and goals. Direct care is provided by the primary care giver, and is available and augmented by the hospice team.

COMMUNITY HOSPICE is a Medicare / Medicaid certified program which accepts patients without regard to age, gender, nationality, race, creed, sexual orientation, disability, and diagnosis. Hospice services are covered by most insurance policies. Admission to hospice is based on the patient’s eligibility and need.
